For New Patients
Welcome to the Vista Dental Care family!
We are excited to meet you! If you are scheduling an appointment with us as a new patient, please read the following important information:
Before calling, please have the following information ready:
- Legal first and last name
- Date of birth
- Insurance Company
- Insurance member and/or group ID
- Employer
- Reason for making the appointment or chief concern(s)
Please note that the New Patient Exam with our office will be a comprehensive exam and DOES NOT include a cleaning. If there are any cancellations or openings for cleanings, we may offer them to you on that same day. Otherwise, cleaning and other treatments will be scheduled after you first appointment.
If you have any x-rays (FMX, panoramic, or bitewings) from your previous office, please have them emailed to us at info@myvistadentalcare.com before your appointment. Panoramic x-rays or FMX (full-mouth x-rays) must be taken within 5 years. Bitewings must be taken within the year and each x-ray must be emailed as an individual file (not a screenshot).
For Returning Patients
Welcome back!
If your last visit with our office exceeds 1 year, we will need to schedule you for another comprehensive exam so that may update your health history, dental records, and treatment plan.
If you have new insurance, please bring all necessary information to your appointment!
Emergencies
Dental emergencies include:
- Severe toothache
- Badly cracked tooth
- Knocked-out tooth
- Extruded (or partially dislodged) tooth
- Dental abscess (swelling of the face or jaw)
- Lost or broken dental restoration
- Severe soft tissue injury (like a busted lip or deep cut)
If you are experiencing any of these dental emergencies, please contact our office immediately. We may be able to squeeze you in for a same-day emergency appointment, but know that there may be a long wait time. Otherwise, call us so we may provide you with further instructions.
In the case that you are unable to get an appointment, please go to an urgent care facility or your nearest emergency room!
